First of all, I don't believe Tarot Cards have any kind of "magic" or mystical powers. I don't have anything anyone who does.

2021.11.30 05:42 MattyInThailand Questions about CAD diagnosis in Thailand (of all places!)

Hello people. I was diagnosed with coronary heart disease in June 2020 at a hospital in Thailand, where I live (although I am British). The diagnosis came after seeing multiple cardiologists in multiple hospitals, complaining to them all that my heart was in bad shape. Each of them ran EKG's, ultrasounds, stress tests and blood tests, a couple of them gave me holter monitors. Each and every one of them would always tell me my heart is fine, I probably just drink too much coffee, stop worrying.
I disagreed with them all but would get spoken down. Over the course of 12 months, my heart would randomly go into these crazy skipped beats, with a very strange rhythm. Fast, then slow, hard, then faint. It was like a pair of old shoes bouncing around in a washing machine (to borrow someone else's description). These events would come and go and when they got bad, I would visit a hospital, always to get told the same thing - my heart is fine. The last couple of months before getting the diagnosis, it got real bad. I couldn't sleep lying down as my heart would go crazy and i would feel like I couldn't get enough air. So, I visited a cardiologist, one I had see three times previously, and insisted on full tests. He told me I was wasting my money but if I wanted to waste a few thousand bucks that was fine with him.
So I had a CT scan with that strange coloring stuff they pump into you. I also had echocardiogram, ultrasound, full blood work and a couple of other smaller, standard tests. It transpires I was right, and I have coronary artery disease with a 25% stenosis in the LAD and 75% in the Ramus Intermedius.
The cardiologist didn't actually tell me i had CAD. He told me it was nothing serious, I would be fine but needed to take some medicine 'just to be safe'. He gave me Bisoprolol 5mg, Atorvastatin 40mg and Aspirin 81mg. It was the nurse who told me on the way out to make lifestyle changes because, she said "You actually have coronary heart disease, it's not really correct when the Doc told you all is well". Change your diet, lots of oily fish she said. Exercise. With the meds, she said, I should be fine.
This is the problem with medicine in Thailand. We do actually have great hospitals over here. Even the poor, underfunded municipality hospitals have all the equipment necessary, but the private hospitals (of which there are many) are stunning. State of the art facilities, extremely comfortable rooms like a 5* hotel. Pretty, friendly nurses who treat you like royalty. The problem is the doctors - honestly, they truly are terrible, for the most part. I was once diagnosed with Prostatitis and took extremely strong antibiotics for 12 months, when it transpired it was a bladder issue and my prostate was fine. It took a UK doctor 5 minutes to tell me that, while there for a quick holiday once.
Anyway, my point is that I have this illness, but dont have great medical support to go with it. In a couple years I will move back to the UK, but for the time being I was hoping some of you guys might be able to answer some questions I have if thats ok?
Just to say first of all I am male, 48 years old, 6 feet tall, 105 kilos (230 pounds)
*Firstly, just how serious is this? I appreciate all cases are different, and you should never ask the internet for a medical consultation, but just basing this as a general opinion - 25% blockage in one artery, 75% in another, is that something to worry about in terms of dropping dead randomly at any given moment? Or as far as CAD goes, is this quite an 'average' illness I have?
*I have made lifestyle changes over the last 12 months. I eat very well now and have cut out all fast food, pizza, pasta. I eat mostly oily fish, couscous, salads, a little rice sometimes. Soda drinks are out of the window and I drink mostly water now. I gave up drinking in Feb this year, although I only ever drank pints of beer before (never top shelf) I have totally quit just to be safe. Will all this prevent the illness progressing? Is there any hope of reversing it a little if I stay committed to eating like this?
*The heart arrhythmias. It has actually improved this year and is absolutely nothing like before. I can sleep lying down now for a start. Im curious to know, do you guys think I might have been experiencing afib last year? You know, the old pair of shoes in a washing machine analogy is such a perfect way of explaining that feeling. Fast, slow, hard, soft all in the space of five or six seconds for around an hour each time. I dont experience those anymore, thankfully but it used to be daily. So what on earth was that, do you think?
*My current status is that i feel ok, but do still have weird beats. They are nothing like before, but they do still prevent me from exercising which I realy want to do. Most days there will be, maybe once every hour or so, a little flutter here and there for no reason. Sometimes after I eat it gets quite bad and freaks me out, maybe for an hour after eating i get these proper 'skipped' beats where every 3rd or 4th beat there will be no beat at all. Beat, beat, beat, skip a beat, beat, beat, beat, skip a beat. Like that. It feels more like it is coming from the upper stomach for some reason but if i put my finger on my pulse, i can literally feel the skipped 3rd or 4th beat (or not feel it). As I said, this usually lasts for an hour after eating, or drinking a morning coffee. Is this anything to worry about?
*Exercise. This is the big problem for me. I really want to get out on my bike, or jog a little, but every time I start i get palpitations/arrhythmias like 10 minutes after starting. Even doing a little work in the garden I will experience this. It freaks me out, i worry i am about to have a heart attack if i keep going, so i stop. As a result, I do live a terribly sedentary lifestyle and spend most of my time at my desk, or on the bed. I realy want to get out there but this worries me. Should I just push through it and accept it as just a thing that happens, but wont hurt me? Is there any reason to worry?
Thanks so much for taking the time to read this. I am worried over here and have no one to ask about this kind of stuff. The doctors are crappy and i have become a bit morbid in my thinking which isn't healthy. I'm always looking at my wife and thinking silly things like i will be leaving her alone in the world soon. I have zero clue just how serious an illness this is and until i move back to UK, I will never know, so if you guys could give me a little feedback i would really appreciate it. Thanks so much!
